Bell Atlantic from Delaware. Apparently the phone was maintained by Bell Of Pennsylvania before both companies became part of Bell Atlantic. This phone started out as a 1C and at some time was updated to 1D. Check out the Bell Atlantic embossed handset. The upper housing lock is a 29A16 and the vault lock is a type 30B.
